The Health Information Management (HIM) Department has moved!
The Health Information Management (HIM) Department, has moved from the Hospital's Main Campus, located at 1 Heroes Way (Formerly 1300 Roanoke Avenue), to 1225 Ostrander Avenue. Patients can request their medical records by completing the “Access Request Form” HIPAA form at this address in the main lobby of the building or by contacting Medical Records for further instruction:
